-1809094991 2013-03-20 My inspection found the weeds and small trees still located around the tank battery. The dike I requested to be built around the tank battery and separator on 10/18/2012 was not built. I called Jamie Campbell at 440-858-7252. He said he did not remember that I requested a dike to be built. Jamie said that he would contact Bob Barr Jr. about the dike request. I informed Jamie that I was issuing a compliance notice. I took photos of the tank battery and separator to document their condition. 9-9-05(A)9
1363959664 2012-10-18 My inspection found this well in production. The well was equipped with a 4" concoil unit. The tank battery contained one, rusty, 100 barrel production tank and one, rusty, 50 barrel brine tank. It was surrounded by a very weathered dike not capable of containment requirements. There was a pond within 25' to 30'. There were weeds, small trees, and debris around the production and brine tank. There was a plug in the load line. The heat treated separator had the correct spacing from the tank battery. A stenciled identification sign was posted on the production tank with half of the required information. The lease road was in good, stable condition and shared with permit number 2-2668. According to our database, the last reported production was in 2010. I called Big Sky at 740-624-0434. I instructed Big Sky to remove the weeds and small trees from around the tank battery. Also, I told them to build the proper dike around the tank battery and separator.
