This well was also initially cited for Urban Rules Violations. Those violations have been removed at this time. NONE_
-1759580161 2008-06-03 Surface Cement Job. Harold Service Co. is doing the drilling. TD 312'. Drilled on fluid. No conductor set. Ran 297' of 7" in a 8 3/4" hole. Using new 7" 17lbs and new 4.5" 9.5lbs. Pit GPS N38.83547 W082.18813. Had good circulation and cement returns to surface. Cement stayed at the surface.
-200896966 2008-10-09 The original location slipped and was not useable. The well was re permitted to its current location. The original site has been reclaimed. The grading looks good seed and mulch was applied. Grass is growing well now. The drill pit has been closed. The well site has been graded. No seed or mulch has been applied to location at this time. The lease road has some drainage issues, there are no water bars or diversion ditches on this road. Sediment from the lease road is being deposited on Mill Creek Road. I talked to Dave Strang in the field and he is going to fix the sediment problem. I suggested a series of water bars and diversion ditches to control the run off. Reworking the apron of the lease road to divert water away from Mill Creek Road would help.

1593827408 2013-08-23 According to the operator this well is providing domestic gas supply since 2009. I will suggest to the operator that they install gas meters to their domestic supply wells to verify that the wells are productive. There are no further issues at this time.

514075792 2008-08-08 Pit is open. Pit liner is falling apart. The pit liner is a clear plastic 4 mil. The liner has become brittle. Pit water is in contact with the soil. Pit is not capable of retaining fluid. Pit needs to be pumped out and water taken to an injection well. The pit needs to be closed. This well site had to be relocated due to the first site slipped. They need to reclaim the first proposed well location when the drill pit is closed. I called Dave Strang and he is working on getting a contractor on site to do the work. The first site GPS is N38.83446/W082.18903. Root balls and trees need to be cleaned up and Vegetative growth needs to be established. The lease road needs some erosion controls like water bars and diversion ditches to help prevent erosion. Lease road entrance off of Mill Creek Road is GPS N38.83410/W082.18389. NONE(A)
