Two inch line from well to tank battery has corroded away and leaked in multiple places. 1st leak in line is 149 feet from wellhead. Gas is leaking from line with enough pressure to trench out a depression in the soil 6 inches in diameter. 2nd leak in line is 263 feet from wellhead. Leak appears to have come from gate valve, and contaminated soils with oily materials in an area 8x8 feet. 3rd leak in line is 406 feet from wellhead. Crude oil has leaked from line and contaminated an area of 10x20 feet. An estimated 10 gallons of crude remain at contaminated area in a small pool. There is evidence of crude having washed over hillside in past and running 15 feet down hill. All producing leases shall be legibly identified in a conspicuous place on or near the well-head or the storage tanks as to owner, lease name, well number, permit number where available, county, and an emergency telephone number. If multiple wells are being produced into the common tanks each well-head shall be identified as to owner, permit number where available, well number and lease name. All oily materials and contaminated soils shall be removed from site and disposed of properly. Contaminated area shall be reseeded and mulched. Defective lines shall be replaced or repaired. Well shall be produced, plugged, or placed in temporary inactive status. I Spoke with Ms. Metcalf on 7/21/11 and notified her of violations on this well. Extension granted is to allow time to correct status of well, either pump, produce, or place into temporary inactive status. Contamination must be cleaned immediately. 9-10-04

-1258787889 2012-11-27 The pumping unit appears operational and has a power source. The production casing and the tubing both feed into the sales line. The valves are both open. There are no leaks along the sales line and there is no sign of contamination along the line.
-1399124721 2013-10-15 Follow up to -1116832857. Contaminated soils not yet removed. There is a new area along the sales lines roughly 100 feet from the last leak where there has been recent contamination. The vegetation in an area roughly 5 x 10 around the line has been coated in oil/fluids. Their was no leak actually noticed in the line. Their is a recent hose clamp around the line.
-357555539 2013-09-11 Follow up to -1116832857. The area along the sales line near the oil leak was showed to me and dug up by landowner. Roughly 2-3 inches below the surface the soil had a black discoloration, slippery feel, and petroleum smell associated with it that was not evident in the soil outside of the original contaminated area. The company has yet to post the well under their bond.
-423955745 2010-07-01 Inspection this day found well producing with jack, ID's at well say #3 and now owned by M-H Oilfield Services. Tank is probably not being used right now, overgrown, oil may be going to tanks down at highway. This was a well Columbus has asked me to check for ownership - this well should have been transferred to John Metcalf with the other Matlack wells and isnow owned by Mark Hines. Will ask Columbus to put on his bond.

-975536672 2013-04-17 Follow up to -1116832857. Well equipped with an electric motor. The valves on the production and the tubing were open. There was no contamination along the sales line. The company still has not placed this well on its bond, and as such has not shown production numbers.
1014805775 2011-11-10 Follow up inspection to original NOV. ID has been put on well but is incorrect. Production line once again leaking a small amount of gas at first area of contamination. Oily materials from second site of contamination removed but no vegetation growing in area of contamination yet. Grass growing in third area of contamination. Materials from tubing being run from a 2" hose into a bucket next to well. A large pile of old absorbent pads seen at well site. Spoke with Ms. Metcalf on 11/21/11 to go over progress and steps that need to be taken to bring well into full compliance.
1539941538 2014-05-09 The stuffing box had leaked and contaminated an area around the well head roughly 5x5 feet. Any faulty equipment shall be repaired or replaced. All contaminated soils and materials shall be removed and disposed of properly. Any affected area shall have topsoil replaced, seeded and mulched, and steps shall be taken to encourage vegetative cover.

The stuffing box is leaking and shall be repaired or replaced. Materials from tubing being run to a 2" hose equipped with a ball valve. Valve was closed but hose just ends.
