All producing leases shall be legibly identified in a conspicuous place on or near the well-head or the storage tank as to owner, lease name, well number, permit number where available, county, and an emergency telephone number. If multiple wells are being produced into the common tank each well-head shall be identified as to owner, permit number where available, well number and lease name. The stuffing box had leaked in the past and released oil onto the ground surface. There was an area of contaminated soils around the well head roughly 5 x 5' and then moving down the hill 25 x 3'. A connection along the 2" steel line leading towards the tank had also failed and contaminated the ground. An area of contaminated soils was noted roughly 5 x 5 ' and then running down hill 60 feet. The contamination appeared to stop short of the creek by 25 feet and there was no evidence of oily materials in the creek. All contaminated soils shall be removed and disposed of properly. All affected areas shall be reseeded and mulched and steps shall be taken to encourage vegetative cover.		9-1-07
