Note: bridle appears crooked. Valve open with 1.25" black plastic routed to 100 bbl tank. No production statements submitted to the Division since 2006. GPS reading obtained for the 100 bbl tank. Chieftain Energy will be notified to install new ID and submit statements of production.

Chieftain Energy Contractor, Mr. Ron McBride and Paul Grim's employee, Mr. Paul Thompson have stated that pump was repaired because it was "salted up." The line from well to tank will need to be connected. The well site and lease road will need to be graded in spring, due to activity and weather. Identification not observed at well and now missing/ incomplete at the well (only permit # shown). Chieftain Energy will be notified of inspection results. GPS reading obtained for well and 100 bbl tank.
