-154588650 2013-12-04 On the day of my inspection, I reviewed the Division of Oil and Gas production records and found them current for the year of 2012, the well is equipped with four and a half inch production casing and a concoil catcher, there is a one inch rubber hose leading from the concoil catcher into the two inch steel production line which leads into a separator / drip (1) two hundred ten barrel production tank (1) one hundred barrel production tank, there is (1) fifty barrel steel brine tank and a poly tank setting inside a containment dike.

1367862705 2013-05-06 Pryor to my inspection I received a phone call from Bobbie with the Guernsey County Auditors Office with a request for a inspection on this well due to the current owner filing secondary production. I reviewed the Division of Oil and Gas production records and found for the year of 2012 has not been posted but the previous years were current, At the well I found four and a half inch production casing standing from the ground with a ball valve, the well is set up with a rabbit stile head and a short rubber hose leading into a steel inch and a half production line, the valves on the well are open, there is no I.D. posted at the well head. The tanks are located at the roadside, there is a separator/ drip (1) two hundred ten barrel production tank, (1) one hundred barrel production tank and a steel fifty barrel brine tank, the valves on the production lines inside the dike are closed, there is no I.D. posted on the tanks. 9-9-02

1395945565 2014-03-27 On the day of my Compliance Follow-up inspection, I placed a phone call to Samurai operating and spoke with a company representative and informed him of the Compliance Notice that was issued on 05/06/2013 and I requested the name and a contact number for the pumper, I informed the company representative that it was important he returned my call and resolve the compliance. The pumper Jim Posey returned my call on the same day and informed me that the well was in production and the I.D. was posted on the tank. The pumper is meeting with me on 04/02/2014 at 9:00 a.m. for my site inspection.
927963516 2012-08-20 On the day of my inspection I witnessed at the well four inch well casing with four inch ball valve and 4x2 swedge with a rubber hose leading into the production line, I also looked at the chart and there was no record at that time, there was no I.D. posted.
