-1116407205 | 2012-10-10 | FOLLOW UP TO NOV # 1350659426 Injection tubing is full of fluid. Unloading pad drain has soil with vegetation growing out of the grate. Both dikes have water in them (6-12 inches). Brine hauling log on site shows only one small load hauled on 9-21-12 from tank compressor 1000 gallon tank. Load was put into # 2 tank. Called John Kent and we discussed the need to keep water out of the dikes. We also discussed the well's condition and the fact that they are not injecting water at this time. John told me that they are planning an acid treatment to help open up the formation. They are hauling their production water to Athens injection well.
-1175157537 | 2012-07-23 | Zero pressure on injection tubing. Last recorded brine transport in log book dated 12-14-11. 1000 gallons from compressor yard. Last recorded company annulus test (monthly test) on 6-4-12.

-1374084100 | 2012-02-17 | Follow up to a verbal compliance notice originally issued 2/14/12 by Jon Scott for water in the dikes at the injection facility. Upon inspection there was roughly a foot of water in both of the containment dikes. Dikes shall be kept empty of all fluids. | 9-9-05(A)9
-1381618046 | 2014-06-18 | Inspected the containment dikes, tanks, unloading pad, injection pressure and annulus pressure. Identification posted at well. Well is connected for injection. The containment dike with the two 100 barrel steel tanks has approximately 10 inches of water in it. I called John Kent and left a message about the water in the dike and that he needs to remove it and keep the dike free of fluids.
-1412153767 | 2012-12-11 | Acid job. Josh Garland MRI 2 was on site. I stopped by to check on the progress. Unable to pump through tubing. Acid started to cause leaks on the pump truck. -1797620057 | 2013-11-21 | Inspected the unloading pad, containment dikes, tanks, injection pressure and annulus pressure. Injection tubing has a valve that is closed. The injection tubing is not connected to injection line due to a well stimulation on 11-15-13. The landowner Mr. Craft told me that acid was spilled on the location during the well stimulation on 11-15-13. Lime powder was observed in multiple locations. Mr. Craft told me that most of the acid went into the dike and was removed with a vacuum truck. I called John Kent and asked about the release. John said that he did not know about it and would ask the field staff about the details. I requested to know the quantity and type of acid that was released and what was done to remediate the site after the release. Both dikes are free of fluids.

1252004373 | 2009-08-27 | Zero pressure on injection. 100psi on annulus. 3storage tanks 6000 gallons each are located in a dike. They appear to be for storage and possible filtering. The dike liner is torn and damaged in multiple areas. 2 100bbl tanks are also located on site in a different dike. This dike liner appears intact. I called Adam Schoppe and discussed the violations and he is going to arrange the necessary work in the field. I told Adam that the liner needs to be 30 mil with 4" sand under and 4"washed gravel over the liner. I also mentioned that they need to check the dike capacity and make sure that it will hold the correct amount. | 9-3-08(A)
1262709163 | 2009-10-13 | Received a call from Adam Schope (Mountaineer) Re: What is the minimum thickness for the liner. I told Adam a 30 mil liner needs to be installed. Adam will call me when they get ready to replace the liner.

1350659426 | 2012-10-10 | Injection tubing is full of fluid. | 9-9-05(A)9
1354126946 | 2012-11-28 | Witnessing an acid job. Three Barrels of a 28% Iron Acid -20M was mixed in the tubs with 6 Barrels of fresh water for the acid treatment. The diluted acid mixture was allow to sit in the mix tub too long and corroded through the welds on the tubs. The mixture then mixed with the limestone along the lease road creating a caustic foam that impacted the area. Roughly 480 square feet of limestone lined lease road was impacted under the truck and roughly 40 square feet of soil was impacted near the well head. The acid ran along a ditch roughly 45 feet before encountering a culvert. The acid does not appear to have traveled through the culvert. Contaminated soils and gravel shall be removed and disposed of properly. If a significant amount of impacted soil is removed topsoil shall be replaced to restore the surface to its original grade. Impacted are shall be seeded and mulched and steps shall be taken to encourage vegetative cover. | 9-1-07
1354287102 | 2012-11-30 | Follow up to inspection 1354126946. The limestone gravel has been replaced. The contaminated soil has been replaced and fresh top soil has been laid down. The area has been seeded and mulched in order to encourage vegetative cover. Landscaping mesh has been laid down over the mulch to ensure the mulch remains on site. The culvert has been cleaned out. The soil in the lease road was field tested for acidity and it registered a pH of 7. Pictures of the reclamation taken. The UIC numbers and log of a Viking Well Services Brine truck on site were also verified.
1354290366 | 2012-11-29 | I called and talked to John Kent Re: what is their plan for getting the well capable for injection. John told me that they are rescheduling the acid stimulation for the 1st week of December 2012.
1355248202 | 2012-12-11 | Company attempted an acid treatment on well. Three Barrels of 28% Iron Acid were pumped into tubing but were never displaced into formation. Pressure was raised to 300 psi multiple times but the acid never left the tubing. The tubing will be swabbed clean and acid will be stored in a plastic tank for future use. The annular pressure was monitored during the treatment and there was no sign of communication. During the treatment the annular space had no pressure on it while the injection tubing had 300 psi.
1359490911 | 2013-01-29 | There is still roughly 1-1.5 feet of water in the dike. The drain on the offloading pad is still plugged and full of water.
