1062106771 | 2013-12-10 | My inspection for Complaint #4200 and #4452 found the small trees in the tank battery cut down and removed per my request. The proper identification signs were posted at the well and tank battery. The overgrown weeds were cut down. A plug was installed on the load line.
1359388765 | 2013-01-28 | My inspection found the well equipped with a plunger lift unit. It had no identification sign. The tank battery contained one, 100 barrel production tank. The identification sign stenciled onto the tank with half of the proper information. There was no plugs on the load lines. The dike was weathered; however, it meets containment requirements. The separator was in good working condition. There was one, 50 barrel brine tank next to a vertical drip tank at the sales flow line. The 7 day production chart was located next to the sales flow line. The well shares the production tank and separator with permit number 2-1699. The lease road was in stable condition with a locked wire steel line at the entrance. This well was placed in production prior to House Bill 278. I called Jamie Campbell at 440-858-7252. I instructed him to install the plugs on the load lines and place an identification sign on the well. I will re-inspect by 2/8/2013.
1393361792 | 2014-02-25 | I was on location today with O.D.N.R. Supervisor Tom Hill to check compliance of Chief's Order # 2014-43. My inspection found the well to be equipped with a plunger lift unit. The demco valve and the valve leading to the flow line on the wellhead are open. There is a 100 BBL production tank surrounded by a proper dike. The heat treated separator has the proper spacing. The flow line valve leading into the separator is currently open. The sales line valve leading out of the separator is currently open. There is a 25 BBL tank sitting 20 ft outside of the tank battery that is not currently being used. There is a second separator on location for the sales line. The sales line separator is connected to a 50 BBL tank. The production chart in the separator and the production chart on the sales line show the well has been produced since the last chart change on 2-24-2014. The proper identification was located on the tank battery. The lease road is in stable condition and is secured by a wire gate. The lease road and tank battery are being shared by permit 2-1699. The well is in production which is in non-compliance of Chief's Order # 2014-43. The well needs to be shut-in immediately. | NONE9
