-1802980523 2012-04-11 I received an email from Bob Gaffney (mailto:blklabantq@yahoo.com) on 4/11/12 as an idle well complaint. I conducted a status check on the site and found that the well head was fitted with a 4-1/2" to 2" swedge and it was open flow to tank battery. The valves on the well head were open. The tank battery consisted of one 210 barrel stock tank, one 50 barrel brine storage tank, one separator and an earthen dike. There were no leaks or spills at the tank battery or well head and proper identification was posted. The chart showed that this well was producing. I met with Mr. Gaffney and explained to him that the well was producing a minimal amount of gas, but since it was actually in production we could not force Vanguard to plug the well. I advised Mr. Gaffney to seek legal council if he wished to further pursue the matter.

-2054076582 2005-11-16 The tank battery is shared with permit 1678 and consists of 1-210 bbl storage tank, 1-50 bbl salt water tank, 1-separator and a low dike.
-445516812 2010-04-29 I inspected the well. The well has been hooked up to the production line. I called Russel Miller(330-506-8152) about the status of the well. He is in the process of getting a workover rig to swab the well down. The office has sent the production on the wrong well. The office is correcting this problem. This NOV continues.

1271355953 2010-04-15 I called Russell Miller of Vanguard Oil and Gas. He stated that they would get a rig into the well next week and swab the well down. Then plumb back the production equipment to produce this well. This NOV continues.
369690676 2010-04-13 I received an email from Mr. Bob Gaffney of 15678 Garfield road, 330-257-4637. He emailed that this well has not been producing and the company pulled the pumping unit last fall. He wanted the well either to produce or plug and take out the tank battery. I inspected the well and found that the pumping unit was taken. The RBDMS shows no production since 2003. A closed ball valve was on top of the 4 1/2" casing. I called Russel Miller of Vanguard(330-506-8152) about the status on the well. I gave Russel a verbal NOV on plug or produce this well. He stated that he would find out what they wanted to dot with this well. He would get back to me. I called Mr. Gaffney back to let him know what I had found out. I would get back to him once I find out from Vanguard. This NOV continues. NONE
