-1432127248 2010-06-08 Meeting with Mr. Mike McPeek, Kilbarger Investments Production Manager to provide list of all wells, in the State of Ohio. The wells located in Hocking and Vinton counties, with a "Well Type" TAAD, HAD and AD were highlighted. The Consent Agreement between the State of Ohio, Division of Mineral Resources Managment, and Kilbarger Investments, requires well heads to be un-earthed to verify annular disposal connection.
-1599457933 2010-04-07 An inspection was conducted with Mr. Robert Stonerock, Oil & Gas South Region Supervisor. During the inspection, it was noted that the AD line remains disconnected, but will need to be removed from location. If the company continues to utilize 100 bbl tank near the well, an earthen dike will be required.
-1707135033 2010-02-05 An inspection revealed illegal Annular Disposal connection. A 1.25" black plastic line was traced from bottom of knockout located on top of nearby 100 bbl tank to the well location. The line remained above ground through tree line and found underground near well. Dug down approximately 1.5' and observed the line routed to the annulus. Records indicate that this well was never permitted for annular disposal. The displayed identification contains the wrong permit number and shows Permit No. 597. Note: snow cover at time of inspection. Contacted Mr. Mike McPeek, Kilbarger Production Manager, at 2:56 pm and instructed to disconnect AD connection immediately and correct identification. Mr. McPeek confirmed they had been filing production records under Permit No. 597. He states the earliest this could be corrected would be Monday. Central Office will be notified of filing error. 9-3-12(A)B

-281789011 2010-03-19 A follow-up inspection revealed that the AD line remains disconnected from the well. The water knock-out tank has been bypassed, AD line cut with installed elbow attached to approx 10' section of 2" steel for fluid transfer to 100 bbl tank located along Walnut Dowler Rd. A 1" black plastic gas line from the casing side of well remains connected to 100 bbl tank containing the knockout.
-986020425 2013-06-25 Production well inspection, this well consists of a pumping unit and 2-100 barrel tanks. The well has a rod broken in it. Kilbargers will have a service rig on the well in two weeks to repair the broke rod.
