Inspection of the complaint area found the line leak had been repaired by the producer. Inspection of the production fluid storage area found obvious signs of where the 100 barrel tank has leaked oil inside the containment dike. With 2- 100 barrel tanks present along with a mechanical separator. The owner shall clean up the oil from inside the tank battery and make the dike sufficient to hold 110% of the largest tank in the impoundment according to SPCC rules and also properly post well identification in a conspicuous place within 10 days. The owner shall also properly identify and mark the entire gas pipeline according to the Ohio PUCO guidelines within 90 days. Mark any gas lines on surface within close proximity to a inhabited dwelling within 30 days. 9-10-03

1370965049 2013-06-11 On 06/10/2013 I received a complaint from the Divisions Uniontown Office. A Gary Hughes was the complain tiff but did not own the land, I asked Mr. Hughes to have the landowner call me. A Dennis Buck then called within 10 minutes and stated He did not own the land either but His Daughter and Son In Law did own the land and were present in the room with Him. A David and Kristine Hammeric of 50751 Elno Ave Akron Ohio 44319 stated that there was a gas leak behind a camper on their property and it caused a mower to catch fire and burn on 05/24/2013. I received no notification from any party or local government agency pertaining to this matter and did respond on 06/11/13 at 10:30 AM and photographed and documented the scene. A inspection of the source well API # 34-111-2-2009-0000 was done on this day and violations issued accordingly to ORC 1509. Monroe County 911 records show a Wayne Parker as the party making the 911 call on 05/24/13 at 21:21 hours.
1371088243 2013-06-12 Notification was received by Kerry Brown in regards to the Compliance Notice #1245563898 issued on 06/11/2013. Kerry stated that the tank battery has been rebuilt to SPCC rules and the oil inside the tank battery cleaned up. Kerry also stated that He properly identified the 2 gas sales line close to any dwelling. Kerry and I discussed properly mapping and identifying the remainder of the 2 gas sales line with the understand that due to dense vegetation and severe terrain conditions this will be a project to be completed during the winter months. We also discussed burying the line close to any of the camp sites such as the initial complaint property.
1372792009 2013-07-02 Status check inspection on resolved Violation found the dike around the tank battery rebuilt and the oil cleaned up with vegetation starting to grow. Proper well identification is in place.
