Water was in dike area at tank but was froze - this well difficult to get to, had to go when froze instead of waiting another couple weeks when road would be thawed out and muddy.

-1606053051 2008-06-18 Inspection this day for yearly pressure test. Well was shut off when we got there. Millard present to perform test, had to fill annulus with 32 gal water - when pressured up to 200#, did not hold and gradually fell to 0 over a period of 15-20 minutes. Millard was afraid it would not hold as had tested it early in the month and had taken it up to 400-500# which he realized later had hurt it's integrety. Must have small hole in casing. Pressure test on tubing while pump running was 530# - this part passes and well is in no danger of contaminating ground water or other formations. Called Tom Tomastik to see what he wanted operator to do but I believe will have to fix casing.

Of 15% - when I checked pressure it was at 530# which is the maximum limit. Carper has also cleaned out storage tanks, dike area is dry but still black from the water running over the tanks and coloring the gravel. Dean thinks it is more black water than oil. Will go ahead and clear NOV with the understanding that they must keep the dike area dry and continue to try and clean it up more. Pressure is passable.
-2013257317 2007-06-06 Completed yearly injection pressure test this day. Pressured up annulus to 100# - pump was not working correctly and couldn't get any higher. Held 100# for 15 min - tubing tested at 318# - also held. Operator says he had pressured up annulus on Monday (this is Wednesday) and when we arrived, annulus was back to 0 already - this well never holds it's annulus pressure, yet feel if there was a pipe or packer failure, we would see a rapid fall. I feel this pipe good but standard test requires 200# for 15 min. Pumphouse had been steam cleaned, looks good, fair amount of crusty, old oil in dike area but no water. Pump running this day

Triad plans to clean out the storage tanks, then remove all equipment from inside the dike area, clean out and re-lay the plastic liner and cover it with gravel. They will then place the tanks back inside the dike, adding a 100 bbl tank for oil/condensate. They will rework the sump pump and collection hole to make it work, will move the pumphouse building off to the side and pour concrete, then replace the building back on top of the concrete. I told John he needed to put these plans in writing and submit to Tom Tomastik. J & L will be doing most of the construction and BBU will be cleaning the tanks. Triad is anxious to begin.

This well still down, looking for hole in the casing but rig will go on new well - they believe they can run packer, tubing and start injecting as they found no new/additional perfs in this well (#7958). Repeated that I needed to be there when packer goes in.

Previously, packer and tubing had been pulled from well, plug set below the perfs and retainer had been set above, well reperf'd above the cement. Retainer and plug provided the opportunity to circulate after casing perf'd to prove well is circulating, zone can be squeezed. Well would not circulate so plans are to get a pump truck in, pressure up on the well to see if circulation can be realized, then cement backside of casing up past where hole is in 4.5" casing. If that doesn't work, will make into a producer in the Cow Run Sand.

I am willing to restart back up, when kill area is reclaimed, on the condition they keep water out of the dike area. Can start back up and test Millard's theory before replacing all the liner. He says he also needs to clean out sump hole and that will help keep dike dry inside.

2021086250 2007-02-28 Inspection this day found dike area full of fresh water. Pump was running, would have liked to put my gauge on well, but valve to shut was not shutting. Contacted operator and asked to put a new gauge on - gauge read 700# and he was sure gauge was not reading correctly - and install a new valve so I could shut to put my own gauge on. No longer gauge in pumphouse - not required. Too muddy to back to well again today, or this week - will try to get a good test next week. Carper W. S. truck unloading water.
