-682921069 2012-11-05 On site to witness testing of flow line and also to apply pressure to annulus of tubing, Sweeny Well Service applied 1000 psi of pressure to annulus of tubing and we monitored pressure for 15 min with no decline which indicated that we have good integrity of production casing and packer is holding fine and also we filled flow line with fresh water and applied pressure to it also with no decline in pressure ,facility is ready to go online once storage tanks are filled with brine.
-698225792 2011-07-19 Stopped by location, found locked gate in lease road and could not get to well - Knox Energy said they would be on location but nobody there this day. Stopped by Constitution and talked to them for a hour.
-959342423 2012-03-05 My inspection this day due to quarterly requirements. I appears nothing more has been done on the storage site - dike wall and floors poured concrete but no equipment inside. I am hearing that well is for sale - several operators have been contacted.
12211028 2013-01-31 Inspection revealed that there was no activity at site today, facility is locked up and I was unable to check pressures today due to fact that owner has not given me a key to unlock gates going to pump house or unloading area.
123687382 2011-07-29 Today, casing pressure at zero when arrived. Rig continues to run packer on tubing to 4300', set packer. Producers Well Service on location, first did pressure test on casing to 2000# - well held and did not drop. Next to do volume/pressure test on zones to see if zones will take water. With packer at 4300', well took water at 2.5 bbl/min at 1000-1200#, water going in Oriskany, Clinton and Medina. Next lowered packer to 4457", below the Oriskany but above Clinton and Medina. These lower zones would not take water - after only 7 bbl, zones were taking water at 1 bbl/min and up to 1850# and still climbing. Records show well was fracted with no notes on staging, baffles, to indicate whether all the zones were treated. I suspect, fract only went to Oriskany zone. Owner satisfied that with salt water, well will take water at lower pressure and wants to put it into production as is. Feels he wants to get some of investment back and can acidize/fract lower zones at future date.
1311767348 2011-07-26 Inspection this day found crew (Berry Well Service) waiting on 2nd fishing tool. Spent 7/22 trying to pull rods, tubing - pump stuck, couldn't get wellhead off, on Monday (7/25) cut wellhead off, tubing dropped 15', 7/26 got fishing tool - overshot size 3 15/16ths, would not go into casing - at this time waiting for bigger overshot tool from WVA. Feel pump is salted in, well has not been worked since 1989. Should come soon and will be able to retrieve tubing, pull well.
1321291544 2011-11-14 Operator had called last week to say they were pouring concrete for the tank pad/containment. My inspection this day found that the pad/dike was still framed up with re-bar but no concrete had been poured.
1336000023 2012-05-02 On site to witness injection testing of well, Producers Services pumped water at a rate of 1.3 bpm and we monitored pressure and we also tested flow line to well@ 1480 psi with a 2% pressure loss and we monitored annulus pressure while pumping and no pressure was recorded it appears that casing has good integrity and packer is holding once testing is complete plans will be made to complete construction of facility.
1339540422 2012-06-12 No recent activity at site since last inspection well is not in operation at this time waiting on surface facility construction.

Mr. Bruce Huffman says they will come in and do another volume test before acquiring. They have plans to do this fairly soon. I was at facility this day and found no activity yet.
735774163 2013-07-24 Accompanied UIC III inspector Dave Ball on inspection of this well and tank battery.
83569378 2011-07-28 7/26 - pulled rods, 7/27 - pulled tubing - found tubing hanger at bottom, started back in with 4" Parmaco set-down packer, 7/28 - finished running tubing, set packer, tested formations for water injectability with Formation Cementing pump truck. Pressured up to 1750#, pump truck not working correctly and can't give it a proper test to see if 4.5" casing will hold pressure. Will have to try again tomorrow. Put 1500# on the well, will see what it reads in the morning. Do not want to proceed further if casing is no good.
99395977 2013-04-11 On site to pressure test annulus of tubing, Sweeny Well Service pumped 5 gal water and inhibitor to perform test they pressured up to 1000 psi and we monitored pressure for 15 minutes with no decline in pressure.

Note: Operator stated that they are not receiving much water at this time.
