The annulus is tied into the flow line, the flow line valve was closed. Took union after the valve apart, put an ODNR DMRM needle gauge on the line, annulus pressure was 10 psi. Operator opened valves, went to tank battery, opened valve to the tank battery, flow line at the tank battery had a leak in it. Per the operator, there is a leak in the 4.5 inch pipe. 4.5 inch pipe needs repaired or well must be plugged. Tank battery, all line leaks need repaired. There is a cement under ground tank, south east corner of the battery, the tank is no longer in use. The tank is partially submerged. Tank needs removed. There is a depression on the east side of the dike, that is allowing water out of the dike. Tank battery dike needs to be sufficient to prevent the escape of brine and oil. Two wells are produced into one tank battery, each well needs it's own identification sign. Tank battery needs to be kept free of water and vegetation. | 9-9-05(A)1 | |||

712717443 | 2010-05-18 | On site in regards to an odor complaint in the area. Well appears to be leaking around the annuals. Gas was bubbling up around the well head. There is a broken bail of straw around the well head. Removed bail of hay, there is a foot of water covering annulus. Called company was transferred to Wally, last name not given. Wally took my information was contacted by Carl, Carl will have personnel on site. Personnel will contact me when they are on site. Suggested to Carl that the well may have a casing issue. Well does have a casing issue, operator has plumbed well into the flow line. Casing needs repaired. |
