-1641415105 2007-10-23 I received a call from LT. Jim McCreary from the Boardman Fire Department about a moving of a tank battery to the C. Smith #1 well site. The old tank battery was moved because a Day Care Center is building close to the existing tank battery. Mr. McCreary asked me to see if the tank battery should be fenced in. It is at the end of a driving range. The new tank is a 600 gallon one. Not the 110 barrel tank. I called George Strawn from Everflow and he stated that Everflow will install a fence around the new tank battery. George said the fence is on order and will be here in around 4 weeks.

-756275851 2007-03-28 This inspection was due to the Boardman Township Fire Departments request. Wellhead and tank battery were GPS. The tank battery also has permit number 2099 in it. This tank battery services 2 wells. ID signs on. There was a Masonic Lodge and a Diagnostic Medical Center 150 feet away from tank battery. There were no violations at this time.
1074870676 2004-01-20 The tank battery consists of: 1-100 bbl storage tank, 1-buried concrete salt water tank, 1- separator and a dike. The tank battery is shared with permit:2099. 9-9-05(A)10
384495872 2012-02-01 I conducted a status check on this site and found that the well head was fenced with chain link and it is equipped with a plunger lift system. The tank battery consists of one 70 barrel poly stock tank and two separators. There was no brine storage tank present. The tank battery was fenced with chain link and the dike was free of vegetation and debris. The tank battery is shared with permit 2099. Proper identification was posted and I found no leaks or spills at the well head or the tank battery. I took new GPS coordinates of the well head and tank battery. There was a drainage ditch cut in one of the dike walls. I called George Strawn (330-727-1614), of Everflow Eastern, to request that dike be repaired. I will follow up next week.
