1083863759 2011-04-27 See Complaint # 11-004. The Uniontown office received a complaint call on 4-18-11 from Wayne Rosenberger (330-862-2203) regarding an idle well on his property. I called Bruce Levengood (cell # 330-340-9645) with Sound Energy on 4-25-11 and discussed the well. I inspected the site on 4-27-11 and found the casing and tubing valves shut in. There is tubing and rods in the well and the pumping unit has been removed. The pump jack pad is still in place. The tank battery consists of 2-100 bbl rusted stock tanks, a vertical separator, and a buried concrete brine vault. There is no legible I.D. sign. The State database shows zero reported production from 1994 thru 2009. Plug or Produce this well and post an I.D. sign at the tank battery. 9-9-05(A)10

1314193525 2011-05-13 I inspected this well for Temporary Inactive Status on 5-13-11. The lease road is accessible. The oil stock tanks are empty, but the concrete brine vault is full of fluid. The tank battery dike is overgrown with weeds and there is no legible I.D. sign. There are rods and tubing in the well, but the pumping unit has been removed. The tubing valves and casing valves are shut-in. The status of this well is the same as when I issued the Compliance Notice on 4-27-11 to Plug or Produce.
