1216i468 2002-05-06 Obtained GPS coordinates. Wellhead contamination approximately 5 feet across. The tank battery has oil and brine leaking from the two inch plug on the salt water discharge line. The dike has been compromised by cutting it on the north end allowing oil and brine to flow out toward the well head. Remove and properly dispose of the contaminated soil north of the dike and all contamination insde the dike. Repair the dike to contain any future spills. Photographs were taken. 9-1-07
1216i881 2002-08-28 The tank battery contamination is partially removed. The water inside the dike continues to register over 6,000 ppm cl. Met on site with Paul Shaw to discuss the existing problems and which have been corrected since our last discussion. Case pends. 9-1-07
1784047638 2008-12-08 I performed a random field inspection. I GPS'd both the wellhead and the tank battery. I walked around the tank battery and the well and found both to be in good shape. The ID sign was posted. There were no violations at this time.
