-1022876443 2006-12-26 Talked with Rex baker on site. He said that they still do not have the water well test results. The sediment fence is installed and is working. They are still drilling the Horizontal portion of the well. I cannot pass the NOV with out the well test result
-132907916 2007-09-11 Vegetative cover has NOT been established on the site. Pit liner pieces are sticking out of the ground. Steep slope on site is starting to erode. No sediment controls are in place. Well is capable of production. Pump jack is operational. Brine tank is connected to the well. A scratched out id is on the tank It is barely legible. Cannot pass NOV until vegetative cover is established.
-1536574462 2007-08-07 A pumping unit has been set. It is not functioning yet, the rods have to be ran. The vegetative cover is starting to grow in patches. The ground is still bare on 80% of the site. Good id. Some work has been done to the fence around the well site. More work needs to be done and more vegetative cover needs to be established before the preliminary restoration is passed. Well is capable of production it is connected to a main line.
-1699603681 2007-09-24 9:00 am Called Bakerwell's office and talked to Rex Baker Re: NOV's and Preliminary Restorations that are past due. Rex said that he never plants grass in pasture fields and did not think that the State had rules to that effect. I informed Rex of his obligations to establish vegetation on all well restoration sites. Rex then informed me that he lost his contractor in the area and is having a hard time finding someone to do the work (plant grass). I discussed my concerns about not mulching the site. I explained how the mulch not only helps to get the grass to grow mulch also helps hold loose soil in place to prevent erosion. Rex said that he will see what he can do in the next few weeks. I called Joe Hoerst and explained my conversation with Rex and Joe agreed to give him two weeks to get some grass growing before we go to the next level of enforcement.
-1842415603 2007-10-05 50% bare ground. No mulch has been applied to the site. This is an urban site. Fencing around equipment and wellhead is made out of wire fence 3'tallwith fence posts and one strand of barbed wire approximately 40" off the ground.
-1893696229 2007-05-23 Some grass seed no mulch. Pieces of pit liner are sticking up. Sediment fence is down on the ground. No id. No fence. Called Rex Baker he said that he is done with the restoration and is not going to do anything else to the site. I will check the site again then time limit is due.
-2052030153 2007-05-08 Dirt work has started they are roughly on grade. No seed or mulch has been put in place. Production equipment is installed. Pieces of pit liner are still sticking out in places. Sediment fence is down on the ground but not removed. Orange safety fence is mixed in with the dirt in several places. No id on site. No fence around well or production equipment.
-942469596 2008-06-09 Well has an urban id attached to the fence around the wellhead. I talked to Dan Fulks (landowner) and he is going to seed and mulch the site to get vegetative cover established in the bare areas.
1176233419 2007-04-05 Production equipment is on site. Well is not connected to the equipment at this time. There is concrete around well between casing and conductor. Concrete was used to fill in the void after the surface cement fell back 20'. No other work has been done to the site. Erosion controls are still in place.
1206019788 2008-02-28 Id sticker on the tank is for an other well. It has the old information crossed out and the current information in small print above it. This is an Urban Permitted Site. This id is not in compliance with the permit conditions. Vegetative cover has not been established at the well site. Bare ground is 80' by 100'. I have called Rex Baker and he said that he is unaware of the urban id requirements. I informed him of the requirements of 1501:9-9-05(E)(1).
1320227430 2007-09-18 Called Bakerwell office and Rex's Cell phone. I left messages on both systems. I told Rex that he needs to contact me Re: lack of vegetative growth,

209342664 2007-01-18 400 psi on gauge. Pit is filled in. It looks like the pit was not pumped before being filled in. Cuttings were displaced and flowed over the hill towards a stream. Rex Baker said that the pit was pumped by Decker well service.
43946916 2007-02-15 The 7" needs to be grouted to the surface. The cement fell back during the surface job. The tool pusher told me that it had been filled in with bag mix but it has not happened. The well head is leaking gas. 9-1-07
666363057 2007-07-23 Dan Fulks has disked the site 2 times and seeded the area. Then he disked the site again to cover the seeds. Some of the seeds have sprouted. I will check the site in a few weeks after it has rained to see if they have any more growth. They did not mulch the site.
805038868 2007-07-09 No vegetative cover on 90% of well site. This is an Urban well and the spud date is 12-02-2007. The restoration should have been done in six months. There are pieces of pit liner , sediment fence and safety fence on the ground. No attempt has been made to divert water from hillside away from site. This is specified in the Permit Conditions. I am sending a copy of the permit conditions and the restoration plan along with pictures of the site to Bakerwell. NONE
835941050 2007-09-18 Site still is 75% bare with no grass. Pit liner pieces are sticking out of the ground. NO MULCH has been applied to the site. Steep areas on the site are starting to erode.
884721523 2009-12-17 Good urban id on fence around the well and pump jack. Fence is not standard for urban site, but is well suited for this location due to being in a pasture field. Location is in use for livestock and it is impossible to determine if the lack of vegetation in some areas is from restoration issues or agricultural use of the land. Mr. Fulks (landowner) did the restoration for Bakerwell.
