-1124100129 | 2014-03-06 | I was on location to follow up to NOV #1140145497. Inspector Dave Claus issued a NOV on 02/16/2006 for the well to be plugged. There has been zero production reported for this well. This well is a potential safety hazard. It is located very close to a college softball field and it is not fenced off. The owner previously agreed to put this well into production and put a new ID sign on the well. The wellhead is currently rusted and the flow lines from the well are open without any valves or bull plugs. This well is currently incapable of production. This well must be plugged. | 9-9-02

-570476571 | 2006-02-16 | FOLLOW UP TO NOV # 1140145497 Received a call from Brendan Dick (cell #330-323-3062) of Walsh College regarding the status and possible plugging of this well. This well is a potential safety hazard. It is located very close to a college softball field and it is not fenced off. The well needs a correct I.D. sign and the well is idle (zero reported production). Plug the well or produce and fence off the well. Jay Cheslock notified Steve Svetlak today of the College's concerns. Svetlak stated that he would address the N.O.V. (idle well, I.D. sign, and fencing) and put the well in production. I will follow-up on his progress.
