2154i133 2002-04-25 Site has not been cleaned up. Site is still in violation. Will contact John McNally (Whidbey Resources) 330-650-6566 and request immediate clean up.
2154i173 2002-05-20 Clean-up has not been performed. On 04-29-2002, John McNally informed Inspector Claus that Everflow Eastern would be taking over the working interest in this well. John McNally stated that he would inform Everflow Eastern and that Everflow Eastern would perform the clean-up at this site. Everflow Eastern (330-533-2692).
2154i187 2002-05-28 Clean-up of this site has not been initiated. N.O.V. still exists.
2154i93 2002-04-03 Random status check on the site. Nothing has been done since the date of the NOV. The violations still exist.
42379508 2002-10-29 The pollution violation and the failed separtator have not been corrected. Repeated attempts to contact Whidbey Resources by phone have been unsuccessful.
8579070 2002-03-14 See Case # 02-010. 3-13-2002 - Received notification from John McNally ( Whitbey Resources); Office 330-650-6566, Cell 330-283-2137; that the separator on the Grace #1 exploded at 0400 hrs. on 3-13-2002 and blew through a fence. Approx. 15 gallons of crude oil drained out but remained in the diked tank battery. No water way was affected. The clean up crew was on their way on 3-13-2002. On 3-14-2002 at 1445 hrs., I inspected the site. Nobody was on location. The bottom of the vertical separator had been blown open and it had been pushed through the tank battery fence. Absorbent pads had been placed on the release area. An area of approximately 20' x 25' was stained with crude oil. The tank battery fence was coated with crude along with some crude staining on the grass to the west of the tank battery. The parking lot to the west of the tank battery had gravel from the tank battery blown onto its surface. No other damage was observed. I also inspected the wellhead. There was leak on the 1-1/2" tbg. at the lower collar of the hammer union, just above the Demco valve. The tank battery area needs to be cleaned up and the leak on the 1-1/2" needs repaired. Also, a check on production records show that production on this well has not been reported since 1993. Production records need to be submitted up to current status. John McNally and Whidbey Resources were called on 3-14-2002 to report my findings, however, nobody could be reached. Another call will be made to inform Whidbey Resources. An NOV will then be mailed.
941907681 2003-03-18 The well is still idle and the damaged separator has not been repaired of replaced.
