When I left site operator was at 80 feet in depth, still in the Sharon, at 80 feet shale cuttings were noted in discharge. Permit posted in the dog house. Eric & Roy Poulson and Bill Keeny on site. Drainage controls in good repair. Lease road stable and in good repair. Pits in good repair. Operator completed lining 2nd pit before I was on site. Pit was in tight clay soil, per Eric Poulson they coated the clay with betionite. Hospital grade muffler is on the mud pump.

-602009804 2013-05-07 My inspection found the well to be equipped with a plunger lift unit. The annulus valve was open and plumbed to the production tank. The well and tank battery are in good repair and are surrounded by a locked chain link fence. The tank battery consists of a 100 BBL production tank, a 100 BBL brine tank, and a mechanical separator. The production tank has spark arrestors and the vents are plumbed into a charcoal filter. The proper identification is located on the tank battery. The lease road is in stable condition.
-664209507 2008-07-30 Operator has installed the lease road to the site. Lease road is stone. Operator has removed topsoil from the drilling location. Topsoil has been pushed to the east. Dry pond has been installed at the south west side of the site. Well stake will need to be re-staked, it was removed during site construction. Pits will be dug when rig moves on site. Bill Keeny was on site at the time of inspection, he said that they dug a test hole for the pits, there was 5 to 6 feet of clay before the sandstone bedrock.

1219344937 2008-08-21 On site at 8:30am, rig and dog house are on site. Operator waiting on sub-base. Pits are completed, working pit has been coated with betinoite clay (dry). Operator lined pit in the after noon, blow pit will be sprayed with betonite gel before lining. Blow pit bottom and sides were completely clay. Working pit bottom had a number of broken sandstone pieces, operator removed rocks. Silt fence is in good repair. Safety fence is up. Permit is posted.
1219677124 2008-08-25 Eric Poulson on site drilling today. Operator has two pits, they are divided with an earthen berm, pit liner from the first pit over laps the east side of the 2nd pit. 2nd pits liner is laid over top of the first liner, 2nd pit liner had slipped, down below fluid level. Operator had made repairs to the liner, before I arrived on site. Liner had been pulled back up, operator was pumping water from under the pit liner back into the pit. Operator is drilling on fluid. Small amount of oil floating on the pits, oil is from residual left in brine haulers tanks. Hole had unloaded, due to the type of drilling being used, operator was drilling on fluid, in which he was aerating, the mix of fluid level in the hole and air was causing the hole to unload. Operator switched over to strictly fluid drilling to prevent this.
1219765931 2008-08-26 Drilling depth 3639'. Lease road is in good repair. Safety fence is up and do not enter signs are up. BOP is in place. Pits are in good repair. South end of blow pit, fluid level is close to the top of the pit. Requested that the rig hands pump fluid level down into work pit. Rig hands began pumping fluid from blow pit to work pit, during inspection. Hole had unloaded on operator during drilling the day before, operator had a backstop constructed that stop any fluid from leaving site. Back stop was still wet from the hole unloading. Hole unloaded when operator was drilling on fluid and aerating. Operator is having issues with the swivel packing leaking this day. When the operator added another drill collar, the rig hands made temporary repairs to the packing, wrapped plastic around it to prevent it from leaking. Rig hands were putting peat down on the rig floor to soak up any drill fluid.
1219859875 2008-08-27 Operator has reached TD. Operator is tripping out drill pipe at the time of inspection. Pits are in good repair. Site is in good repair. Log truck will be on site at 11:00am, production pipe cemented at 5 or 6pm.
1220029024 2008-08-29 On site to check the status of the safety fence. Safety fence is up, I repair two sections that had been knocked down. It appeared people were short cutting to the parking area, there are two yellow jacket nest, next to the area made for people to walk threw. Pits are in good repair, fluid level is down 1.5' south end of the pits, low end of the pits. Allen from Summit on site during inspection, he is going to put up caution tap around the pits, the location has safety fence around it. Allen will have the last pump truck that leaves the site, close the safety fence at the lease road. Pop of and gauge have been put on annulus, annulus pressure was zero.

Silt fence is in place. Lease road is stable and constructed out of durable material. Site is stable. ID sign is up at the entrance to the site.
1227713108 2008-08-25 Mike Williams on site for BOP pressure test. BOP pressured up to 260 pounds and bled off to only 252 in 15 minutes. This was only a 3% lowering.
1271254617 2010-04-13 On site with Jay Cheslock Regional Supervisor and Tom Tugend Deputy Chief of DMRM, Dike has a drain pipe in it. There are no caps on the inlet or discharge of the pipe. Water was dripping out of pipe at the time of inspection. Drain pipe is to be removed, dike needs to be repaired to the point that is sufficient to prevent the release of oil and brine. Silt fence can now be removed, vegetation is sufficient to prevent offsite erosion. Dry pond outlet needs maintained. Lease road was in good repair. Identification sign was posted. Man doors were locked. NONE(B)
1274299570 2010-05-19 Drain pipe in dike now has a bull plug in it. Called operator, he can now remove silt fence and dry pond. Tank battery man door was locked. Lease road was in good repair. Identification sign was up.
1696758743 2008-09-03 I witnessed the perforating and the fracing of the well. I was with Jay Cheslock, Norburt Lowder, and Mike Wilson all from the ODNRM. I walked around the location and found the pit liners were in good shape with the fluid level down 4 foot. There were no violations at this time.
177316511 2008-11-24 Site is snow covered. Emergency number on ID sign is correct and active. One 100BBL tank and a 140 production tank in battery. 8' chain link fence around well, well head has a 6' chain link fence around it and three strands of barbed wire. Separator is 11 feet from the production tank. Fencing around the well head is 3 feet 11 inches south side all other sides are more than 4 feet from the well head. Lease road gate needs, lock. Called company pumper on site at the time of inspection, will put lock on. Well is a rabbit well. Spark arrestors in place, charcoal filter in place.
