-1016255905 2013-04-02 I witnessed Universal Well Service cement the conductor casing for Pander Trucking. Wildcat Rig drilled a 22" hole to 94' and set the 16" conductor casing at 86'. Universal broke circulation and pumped 380 sacks of cement. They displaced with 16 barrels of fresh water. I did not witness any cement returns to surface. Universal placed 25 bags of cement down the annulus. The cement was tagged more than 15' down the hole. Pander Trucking ordered 8 yards of cement, which was placed down the annulus. Cement was tagged at the surface.
-1036683364 2013-01-07 The well owner applied for a reissued permit. I inspected the site as part of the pre-permit review.
-1803176271 2013-03-26 I inspected the well site and found no violations. The construction contractor was working on the well site, while drilling equipment was moving on location.

Universal broke circulation and pumped 250 sacks of cement. They displaced with 39 barrels of fresh water. I witnessed 24 barrels of good cement returns to surface.
