1247049284 2009-07-07 I performed a preliminary restoration status check. There was fluid coming out of the 2 inch line on the casing. Gas and fluid. The fluid would dry up around 4 feet from the wellhead. I Quantabed the fluid. It was 660 ppm. I called Todd Suckow (330-506-2531) from BOCOR. He would come out. I met with Todd and Todd agreed that there was fluid coming out of the 2 inch line. He closed the line. They would monitor the pressure of the back side of the casing. They would connect the back side to the production line if it would be necessary. This case continues. 9-1-07
1272660604 2009-04-27 I performed a preliminary restoration status check. The drilling rig and the debris have been removed. The pits were solidified and closed. The top soil is still in piles on the location. This well does not pass preliminary restoration.
1304167140 2009-04-02 I witnessed the cementing of the 8 5/8" casing. There were no violations at this time.
339566815 2009-08-11 I inspected the backside of the wellhead. BOCOR has hooked up the backside to the production line. This case is resolved.
670033809 2009-08-04 I performed a NOV follow-up. The backside was still closed. Some gas and fluid was coming out around the hammer union of the wellhead. I called Todd Suckow(330-206-2531) and told him that the backside line needs to be installed to the production line. He would have it fixed by next Monday, 8/10/09. This case continues.

The ID sign was posted in front. The location was in good shape. The drilling depth was 387'. They were drilling on fluid. There were no violations at this time.
