-1998370350 2010-11-30 On site at the request of the landowner. Hatches on top of the tank battery are open. There is an odor of gas and oil in the area. Contacted company requested hatches to be closed and secured. Hatches were closed at the time of inspection. Latch on the man door to the wellhead has not been repaired, door is not able to be locked. Man door latch needs moved or the door needs leveled so that is can be locked. 1163685523 2006-11-15 Operator has reached TD. Pulling drill pipe at the time of inspection. Pit water level OK. Operator has failed to maintain silt fence around the site. Heavy rains in the forecast for the night. Operator has failed to maintain safety fence around the pits. Mike from John D Oil was called and informed of the Violations Silt fence needs repaired and installed immediately. Silt fence needs to be repaired and installed correctly and maintained until site is reclaimed. Safety fence needs to be installed and repaired, and is to remain until pits are closed. NONE
1163774828 2006-11-17 Dirt on the roads, rig had moved off. Safety fence still not up. Called company, asked them to get equipment on site to start cleaning roads. Informed them that drainage controls need repaired and safety fence needs put up. Company stated that they will be solidifying pits ASAP.

Urban permit requirements are agreed with at the time of the presite inspection, options to the site plan need to be addressed at that time. Once the permit are set, requirements need to be followed. This site is adjacent to a State Park and a large body of water. Continued failure to follow permit condition on drill sites will result in future permits being suspended.
1288378701 2010-10-29 Received a phone call from Mr. Silver regarding John D. Oil & Gas company being habitually late with royalty payments. Mr. Silver expressed frustration with having to contact John D. Oil and at times involve his attorney to receive royalty payments. I informed Mr. Silver that royalties are a civil matter, I would complete an inspection on the location checking for ORC 1509 compliance. Mr. Silver also voiced frustration that he had a difficult time having the company repair his driveway. Site at this time is not in compliance with HB-278. Charcoal filters and spark arrestors need installed. Board on board fencing is starting to fall apart. Hasps and locks are out of line and man doors can not be locked. Contacted company requested fencing be repaired as needed. Locks be replaced and the site needs to come into compliance with HB-278. Lease road is stable. Spacing is correct. Identification sign is posted and is correct. Wellhead is in good repair. Tank battery and dike are in good repair.

967666414 2013-10-21 My inspection found the well to be equipped with a plunger lift unit. The well and tank battery are in good repair and are surrounded by a wooden fence. The tank battery consists of a 100 BBL production tank and a 100 BBL brine tank. The tanks are vented through a charcoal filter and into spark arrestors. The heat treated separator has the proper spacing and is surrounded by a wooden fence. The proper identification is located on the tank battery. The lease road is in stable condition.
