-1027969118 2011-01-18 Tank battery has been roughed in. Well is a rabbit well. No fencing has been installed at this time. Site had over a foot of snow on it.
-1033385579 2010-11-01 Rig is moving on site. Silt fence is up east side of site. Lease road is rocked to location. Safety fence is on location. Operator is digging pits, pits where dug in dry, tight clay soils. Informed operator conductor and surface must be drilled on fresh water or fresh water mud. Operator may drill shale on air, will need to go to fluid drilling top of the lime. Permit was on site.
-1289707101 2010-11-10 On site for long string cement. Well TD at 4142'. No gas encountered during drilling. Operator was unable to get logging tool to TD was only able to get logging tool to 3950', well was not logged. Well will be logged through pipe. Operator has 3145.3 feet of used 10.5 lb pipe and 1055 feet of new 10.5lb pipe on location. Operator ran 975' of new pipe for a total of 4132' of 4.5 inches of casing in the hole. During the drilling of the hole, the bit laid down at 4000', operator had to trip out of the hole and replace bit. Cementing water had a pH of 7. Operator used 4 centralizers. Operator broke circulation and ran 6 bags of gel ahead of cement operator ran cement displaced and locked plug down with 1000psi. No lose of circulation noted during cementing.
-1483059918 2010-11-02 On site for the end of CND job. Operator called me three times in the morning to confirm depth of conductor and to get approval to set one joint. There was 20' of overburden then shale. Operator set CND 20' into shale. Hole #1 was 17.75 inch 40 feet in depth. CND set at 40' new R-3 pipe, set open ended. 3 Bbl return to surface. Operator did forget to inform me of the time of cement job. Cementer's were on site at the time of the call to me. Operator was verbally reprimanded during inspection about notice to the inspector for cement jobs. 11/03/2010 operator did contact me at 4:25 am to inform me that the surface job will be at 11:00am this day.
-634375273 2010-11-05 Drill depth 2544' drilling on fluid. Pits are in good repair. Liner is in good repair, small rip 6" at the top, water level 4 feet below rip. BOP and air body in place, no leaks noted. Choke lines in place. Site was clean and in good repair. Safety equipment was being used by crew. Safety fence around pits was up. Lease road was stable.

1288790116 2010-10-27 Wad Pol contacted me, operator will start cutting site on Friday and continue work over the weekend. Rig will be moving on Monday, pits will be dug once rig is on site.
1289243384 2010-11-08 Drill depth 3970' operator has approximately 200' until TD. Operator is drilling on fluid. Discharge on the discharge line is 60' from the rig. Pits are in good repair. Liners are in good repair, rip has not changed, fluid level is 4 feet below, fluid line has not came closer than 2 feet from rip. Operator has scrapped mud off of site and stockpiled it to the south and west of the location. Drainage is contained on site, there is a lip around the location containing drainage. Lease road is stable and rock to site. Informed rig no burning of trash on location, rig has complied. BOP and air body are stable little movement while drilling. Port- a jon on location per HB-278. Safety equipment being used by crew (hard hats.) Safety fence up on the south and east side of the pits.
1310387751 2011-07-11 Well is in production. Well is a rabbit well. Spacing is correct. Wellhead needs to be fenced. Stone for the lease road has been removed, depression remaining needs to be backfilled and reclaimed. Production tank does not have a vent. Hatch needs secured at all times. Load lines require locks. Silt fence needs to be removed. Safety fence and T post need to be removed. E-mail sent to operator requesting corrective actions.
131859124 2010-11-08 Gary the tool pusher contacted me from the rig. TD will be tonight, will call when they call out cementers and loggers.
1406821449 2014-07-31 My inspection found the well to be equipped with a plunger lift unit. The well and the tank battery are in good repair. The tank battery is surrounded by a locked chain link fence. The tank battery consists of a 100 BBL production tank, a 50 BBL poly brine tank, and a mechanical separator. The production tank is vented through a charcoal filter and into a spark arrestor. The proper identification is located on the tank battery. The lease road is in stable condition.
379348414 2010-11-03 On site for surface cement. Operator drilled to 407' 11 inch hole. Operator set 379' + 12 LJ of 8.625 pipe. Pipe was new R-3 20lb pipe. Berea Sandstone top 220' to 280' in depth. Operator broke circulation conditioned hole. Operator ran cement, Class A 3% CaCl 126 sacks mixed with 400lbs of Dow Flake and 90lbs of cotton seed hauls. 4Bbl of good cement returned to surface. Cement fell back 6 feet, had rig hand shovel returned cement back down the annulus space. Tested fresh water before cementing pH was 7. Weighed cement during cement job with hauls and flake mixed in, 15lbs/Gal. Cement will be topped off during the long string. No show of gas during drilling. Good circulation at all times. Pits were in good repair. Fire extinguishers on the rig were current and charged. Safety fence was up around the pits. Lease road was stable. Site was dry. Permit was posted in the dog house.
