-2131626824 2013-02-11 There is a 8-5/8 inch inside diameter casing ending at the surface with a approximately 2-inch production tubing rising about four feet above the ground surface. For production tubing protection a thick gauged PVC pipe was placed over the tubing rising another foot above the top of the tubing. Gas and brine was observed leaking out of the top of the tubing. Brine was flowing from under the PVC pipe and flowing from the wellhead. There is a brine scald area 12-feet long and 2-feet wide coming from the pipe. Low concentration Quantab (30-600ppm) measurement was above 600 ppm but confirmed chloride containing fluid was coming from the well head. According to 1501:9-9-05(A)1/1509:20 the well is insufficiently equipped to prevent the escape of gas and brine is contaminating the ground surface. Repairs must be implemented to mitigate this problem. Well must be sufficiently equipped to prevent pollution and contamination. All contaminated soil must be removed and disposed of properly. The scald area impacted by brine shall be seeded and mulched and steps shall be taken to encourage vegetative growth, There is no tank battery associated with this well. 9-1-07

-900660419 2013-02-11 Accompanied Morgan Button MRI 2 on inspection. No identification. Wellhead not visible at the surface. A tractor tire and rim has been placed over the well. Fluid was observed below the rim approximately 12" down.
1313545698 2006-04-20 Can see the casing head at ground level but well head removed; post stuck in hole with tractor tire surrounding it.
1471099012 2014-04-23 Compliance notice follow-up. The well is not in compliance, there is no legible identification.
1595893386 2013-02-11 Well identification sign is missing at wellhead. According to 1501:9-9-05(A)10/1509:23 the well identification sign shall include owner's name, county, township, well name and number, permit number for the well, and emergency contact number. There is no tank battery associated with this well. Refer to inspector's report # -2131626824 for further information on the compliance concerning the leak at this well. 9-9-05(A)10
