Upon investigation, found something had been leaking and oily patch about 6' around well and starting to run downhill. Well is producing with jack on timer and called operator - told him he had a leak and was coming out to fix it or shut down.

1108720460 2013-07-24 Well has a pumping unit on it. There is no horse head or bridle on it and the well hasn't been pumped for a long time. This is evident due to the amount of oxidation on the equipment and plumbing. There is no identification on the well head and the identification on the tank shows the wrong permit number. There are no pollution and contamination issues at the well or tank battery. The tank battery consists of one 100 barrel tank. Production records have not been submitted since 2008. Well needs to be placed back in production or plugged, production statements need submitted, and correct identification needs to be placed on the well and/or the tank. 9-9-05(A)10

1249926489 2009-03-18 Inspection this day found well producing with jack, operator says not making much but valves open, asked operator to smooth ground around jack, fix erosion down below jack since they have dozer out on lease at this time. Good ID's at tank, no dike.
165630520 2014-06-17 Upon inspection, it was observed that the pumping unit for this well is missing the horse head and bridle, and the well is shut in. There is a 100 bbl. production tank for this well with some fluid in it. Identification information on the production tank has the wrong permit number on it (listed as P# 27958). I left Mr. Ryan Carper a voicemail at 10:07 a.m. inquiring about plans for putting this well back into production or plugging it. He has not yet responded.
1755238660 2005-05-19 Re-inspect this day to see how oil spill area coming. Still needs more work - area about 10 x 20 running downhill in pasture, where pumpjack has leaked - has been covered with Gator or sawdust but needs to have contaminated soil removed and seeded and mulched. Called company and told them - said they would get right on it when cleaning up disposal pit area. Will not write up at this time.
