-1551822884 2005-05-06 Received a cll from Patrick Duncan with P&S Environmental Consultants reporting a gas leak at the Athey #2 location. The well was plugged by Petrox in 1995 and appears to be leaking around the wellhead. The area is under water and gas is bubbling through the water. There is no sheen or any odor associated with the bubbles. I contacted Duncan and requested that the water around the leak be pumped out so that the leak source can be seen.
-2072464879 2005-06-03 The well head has been uncovered and bulk cement was placed in the hole. The ID plate was attached and the hole backfilled. The replugging was not witnessed by any ODNR personel.
