-1035543794 2004-10-26 Follow-up on NOV, In-Compliance.
-1394021000 2008-10-14 A follow-up inspection revealed compliance.
-1423770257 2013-06-27 Danny baker along with Clint poling removing the oil and brine from the dike. They will continue to work on 6/28/13 to finish the clean up.
-1467330966 2008-09-12 A follow-up inspection revealed non-compliance. No identification was observed at the well. Pollution and contamination still exists inside the dike.
-1487046234 2013-11-06 Compliance notice follow up, the dike has been cleaned up. This satisfies the compliance notice.
-1867661407 2004-08-24 Follow-up on NOV, Non-Compliance, no action to comply.
-343101355 2014-05-09 Upon inspection, it was observed that one of the 100 bbl tanks for this well has filled up and overflowed a significant amount (approx. 3-5 bbl.) of oil and brine into 2 pits in the containment dike. This well was cited for the same problem on 7/11/2008 and again on 6/26/2013, which indicates that this is a recurring issue. In addition, the stuffing box on the well head is leaking and causing contamination of the soil surrounding the well head. The oil leaking from the stuffing box is flowing downhill and has made it approximately 20' away from the well head. Identification information for this well is illegible and the I.D. tag still refers to Mark Schmelzer as the owner, which is incorrect. The owner of this well is Jeanie Enterprises LLC. Both Jeanie Enterprises and Mark Schmelzer (now the well tender for Jeanie Enterprises) have been notified of the situation via phone call and/or voicemail. Photos have been taken of the contaminated areas on the well site. 9-1-07

1372259997 2013-06-26 Production well inspection, this well consists of a pumping unit and 2-100 barrel tanks. Jeanie Enterprises has allowed the tank to spill over. They are storing oil in a pit adjacent to the tank battery. I have contacted Joe Smeltzer to correct this problem. 9-1-07

1400528602 2014-05-19 Upon inspection, it was observed that this well is not yet in compliance. The majority of the free oil in the containment dike had been pumped into one of the 100 bbl tanks that was not over-full using a gasoline powered pump. The 100 bbl tank that overflowed still has no bull plug in the top bung to prevent further leakage. It appeared that some soil had been removed from the dike by shovel, and oil absorbent bibs were in place to help absorb more of the free oil remaining in the dike. There was still a measurable amount of oil in the containment dike that needs to be removed. The contamination issues still exist at the well head. New identification placards have been placed on the tank battery, indicating that 4 different wells share this tank battery. I called Mark Schmelzer and there was no answer, so I left a voicemail requesting that he make time to meet me at the well site and discuss further cleanup efforts. I also spoke with the landowner, Larry Haren, and he stated that he is having his driveway paved at the beginning of next week, which would have the lease road blocked for the week. In order to avoid further delays in cleanup, Jeanie Enterprises will have to mobilize equipment to the location this week.
1418864346 2008-09-29 A follow-up inspection revealed non-compliance. Contamination and pollution was found in the earthen dike. The bottom section of this dike shows signs of leakage. A bare spot measuring 25' x 32' was observed directly below this earthen dike. At the time of the inspection, the well is still down. A new pump jack has been placed on the location.
1709179014 2014-05-20 Spoke over the phone with Danny Baker from Jeanie Enterprises at 10:44 a.m. regarding the status of the cleanup at this well. Danny was en-route to this well at the time of our conversation to finish cleaning the rest of the free oil and removing the contaminated soil from the containment dike. I instructed Danny to install a bull plug in the top bung of the 100 bbl production tank which leaked, and to fix the stuffing box on the well and remove contaminated soil from around the well head. Danny stated that he would accomplish these tasks over the next few days.
1810055106 2014-06-04 Upon inspection, it was observed that all of the noted violations at this well location have been resolved. This well is now in compliance.
182388024 2008-06-27 A complaint was received by the DMRM. An inspection revealed that new identification would be required. The company was contacted by telephone on 7/11/2008 and informed of the inspection results. 9-9-05(A)10
575352753 2014-05-22 Danny Baker from Jeanie Enterprises was working on the cleanup of this well when I arrived on the location. Danny used a backhoe to remove the rest of the contaminated soil from the containment dike at the tank battery and around the well head. He also used the backhoe to build a dike around the well head. A bull plug was installed on the 100 bbl production tank that leaked, and all new identification placards were installed on the tank battery. The well is currently shut in, and will remain that way until all cleanup and repairs are finished at the location. I will check back next week to see if the leaky stuffing box has been repaired and that the cleanup is finished.
