-250209884 | 2012-09-20 | On the day of my inspection I found that the well head was equipped with a pumping unit and connected to a tank battery that consisted of one 100 barrel stock tank, one 30 barrel poly brine tank, one separator and an earthen dike. Proper identification was posted and I found no leaks or spills around the well head or tank battery. This well shares a tank battery with permits 6819 and 6820. No violations at this time.

1397499607 | 2014-04-14 | I received notification from Eclipse that they had found a flow line leak at this well. When I arrived at the location I met with Dale Smith, who was in charge of the clean up. They shut the well in and identified the leak in the flow line.. The crew installed temporary clamps on the line so they could flush it out with water. There was an estimated one barrel of oil that escaped the flow line. There were two areas approximately 10 ft in diameter where the oil had pooled in a grassy field. I did not observe any contamination beyond those two spots. The crew was in the process of removing all of the contaminated soil and transporting it back to their yard in order to dispose of it at an approved landfill. Dale Smith claimed that they were going to replace the line once they got the soil out of there. I will follow up when he notifies me that the clean up is completed.
1399295125 | 2014-05-02 | On the day of my inspection I found that Eclipse Resources had a crew on location finishing the clean up of the flow line leak. They had the leaks identified and repaired. They were removing the last of the contamination and preparing to restore the affected area.
1399299380 | 2014-05-05 | On the day of my inspection I found that the contaminated soil had been removed from the site. All equipment and debris had been removed from the site. The soil was graded out to the approximate original contour.
1740748101 | 2014-04-21 | On the day of my inspection I found that the well was shut in and the existing flow line had been flushed and clamped. There were no active leaks. There were oil absorbent pads spread around the area in the standing puddles of rain water. The pads were mostly clean. The only water that I found that still showed some oil was down in the ditches where the flow line leaks had been dug up. They had absorbent pads in those puddles as well. The contaminated soil had been removed from the site and there was an earthen berm in place to prevent any residual contamination from escaping the immediate area.
