-1477003809 2008-06-12 RESOLVED WELL IN PRODUCTION
-796464411 2014-02-14 On the day of my inspection, I reviewed the Division of Oil and Gas production records and found them to be current for the years of 2011 and 2012; the well is equipped with a pumping unit above an eight and five-eighths inch surface casing with four and a half inch production casing and two inch tubing. Upon inspection, the pumping unit was not operating. The valve on the four and a half inch production casing as well as the two inch tubing at the well was in the open position. The two inch tubing leads to a chart recorder / drip / separator, and (3) one hundred barrel production tanks sitting inside a containment dike. The valve going into the separator was in the open position, and the valve from the separator to the production tanks was in the closed position. Upon inspection, it was observed that although the valve from the separator to the production tanks was in the closed position, fluid could still be heard flowing through it and into the furthest of the three production tanks. The date posted on the chart recorder was February 12, 2014. There was no identification information posted at the well or the production tanks.

The requirements to meet the Division of Oil and Gas standards are as follows: 1.) Properly display identification information at the well and/or tank
