1259937555 2009-12-04 I performed a NOV follow-up. Nothing has been done yet. I went to the Energy Resources of America office and talked with Joe Glista and Kelly Koval. Mr. Glista stated that the 50 barrel plastic tank will be replaced and the dike area cleaned up tomorrow. This case continues.

1431358545 2011-08-12 Received call on 08/12/2011 from Austintown, OH Fire Inspector Lt. Rick Millirn (330-799-8502) in regards to homeowner complaints of gas odors from well. I inspected well head and tank battery and walked local area. Did not notice any signs of leaks or spills and did not detect any odor related to oil/gas leaks. Spoke with Joe Glista (330-953-1813) of Energy Resources America on 08/12/2011 in regards to complaint. Glista reported that he had a truck on site that morning to collect oil from stock tank. I relayed this information to Lt. Millirn via voicemail. No violations at this time, complaint resolved.
1598357322 2009-11-25 I performed a well inspection. The50 barrel plastic brine tank inside the dike has a 4 inch hole, 12 inches from the bottom. The brine inside was leaking outside the tank. I drove to the Energy Recourses office in Austintown and talked to Joe Klista Jr.(field superintendent) about the hole in the brine tank. He would get a water truck to vacuum out the brine and replace the tank. I would check back next week. This NOV continues. 9-1-07
