-329144086 2010-09-01 I called John Kent on 9-1-10 at 9:57 am, and we talked about the well being idle and that they need to establish vegetative cover on entire well site. John told me that they are working to get the well back in production. I requested a written time line as to when the well will be in production.

39378733 2010-08-31 No Identification. Well is disconnected from production line. All valves are shut. Steel tank has been removed from location. Vegetative growth has been established on location except on two areas 1st scald area is 17' by 10'. 2nd scald area is 12' by 6'. Pictures were taken. A search on RBDMS revealed no production records past 2007 and that was recorded as 0.
621567067 2005-08-03 There were no violations.
739296091 2009-11-05 Well has no identification. Well is disconnected from production line. A drip is located 5' from the well and is connected to a 1.5" orange plastic line, which is then connected to a small gas meter (houseline). Gas is being fed to the house line off of the main 2" steel production line. There is an steel rectangular open top tank on location. The tank is mounted on skids. There is a brine scald area 20' by 15' around the tank. The tank is 3/4full of brine. The Valve on the tank is leaking a small steady drip on to the ground. This leak appears to be the cause of the scald. I tested the drip area below the valve with an High End Quantab it maxed out the Quantab. I have talked to Adam Schoppe (Carl E. Smith) he told me that the tank was used in a recent clean out of the well. After our discussion Adam told me that they will haul the water out of the tank and remove the tank. Then restore the site. Remove tank from location. Restore location and achieve vegetative cover. Post Identification that includes Company Name, Permit number, Lease name, County, Township, Emergency contact phone numbers. 9-1-07
