-356367325 2014-06-18 Appalachian Wireline Service set a 2" inch wire line plug at 530 ft and logged total depth of the 2-3/8" inch diameter tubing at 578 ft. Well is to be plugged Thursday. 6/19/2014
1403183463 2014-06-19 Berry Well Service ran 1"inch diameter tubing to 530 ft and circulated the hole and completed a level full test for 5 minutes to make sure that the wire line plug is holding. Next Cementco Cementing set a plug from 530 ft to 280 ft. The fluid in the well does not appear to be holding so I had Berry Well pull all of the tubing out after setting the bottom plug. Next advised them to let the plug set for a few hours and see if we can tag the top of the cement. If the cement top can not be found they will have to set another wire ling plug and test it to make sure it holds.
1956292380 2014-06-19 On the date of inspection I accompanied ODNR Inspector Mark Moreland to witness the plugging of the well. Refer to Mark Moreland's report for further details.
