-1548205302 2006-06-01 There was no injection pressure on the well. There was pressure on the annulus side but the gauge was not reading correctly. Could not removed gauge to check pressure. Left note at office to replace gauge. Remind operator that restoration of the injection well site must be completed after wheat is removed from field. There is grading that needs to be done to level high areas and fill low areas holding water. Topsoil should be replaced. As previously indicated by the operator, the injection line from the plant to the well would also be buried at that time. -302241433 2006-03-20 Spoke to operator Tony on site. The landowner has requested that completion of reclamation on injection well site and burial of injection line be done after wheat is removed from field this year. Drainage tiles in the area of the injection well will also be repaired/replaced at that time. The operator is collecting seismic data in the area for other possible well sites. Injection pressure was approximately 300 psi and annulus pressure was approximately 800 psi.

1109780347 2005-03-02 Spoke to John Powell of Bigard & Huggard Drilling at site. Well has been drilled to final depth and logging is being done. Picked up samples of drill cuttings for Geological Survey. Cementing of 5 1/2 inch pipe will probably occur sometime after midnight. Gave John phone numbers for Jeff Fry for contact since inspector will not be available for cementing. John will fax cement tickets for 9 5/8 inch pipe and 5 1/2 inch pipe to Bellefontaine office next week when he returns to office.
1118940327 2005-06-16 Observed tubing and packer before being placed in hole for completion of injection well. Packer was Baker Lockset. There were 94 joints of new 2 3/8 inch tubing used for the construction. Packer was set at depth 2957, bottom of 5 1/2 inch casing was depth 3020 and top of Mt. Simon injection formation was depth 3046 according to drilling records. Allowable injection pressure is 940 psi. There were 60 barrels of rust inhibitor brine water pumped into the annular space between the 2 3/8 and 5 1/2 pipes. Pressure test at 1020 psi was held for 15 minutes with no noticeable loss of pressure. A 6 foot diameter culvert with a cement bottom was placed around the well, below ground level. The culvert was filled with gravel and a pipe inserted in the gravel. This is for containment and removal of any potential spill.
1131547308 2005-11-08 Spoke to operator Tony on site. The pit for the injection well has been pumped dry and the water disposed in the injection well. The pit will be backfilled Friday and topsoil replaced. It has not yet been determined when the surface line to the injection well will be buried. If the line is to remain exposed over the winter, a pressure test of the line must be done with the inspector present. When the injection line is buried, a pressure test of the line must be done with the inspector present.
1132762597 2005-11-21 Pit for construction of injection well has been backfilled. Material used to fill pit is approximately 2 feet above existing ground which will allow for settling of material. Since the spud date for the well was 2/17/05, the operator should have completed the restoration of the well site by now in accordance with 1509.072 which allows 9 months for the grading, resoiling and seeding of the site which was disturbed and is not required for the production of the well. Since November is not an appropriate time to resoil/reseed the site and the area is used for agriculultural production, the operator will be given additional time to grade and resoil the site, but it must be completed before planting in the spring of 2006.

The operator indicated that reclamation of the site would be done when the wheat was removed from the field. Wheat is now gone. Areas around well need grading to remove high areas and fill low areas. Site then needs to be properly revegetated. The pipeline from the plant to the injection well also is supposed to be buried and the line pressure tested. Request operator contact inspector at 419-599-0058 to discuss proper reclamation of site.
1166729715 2006-12-21 Injection well was not accepting fluid. Tubing pulled and found that lower joint was plugged with sediment. New joint replaced obstructed one. Well was circulated to remove additional sediment. Thought that sediment came from storage tanks. A new Lockset 2 3/8 x 5 1/2 inch packer was returned to the previously set depth of 2957 feet. Sixty barrels of water with 8 gallons of rust inhibitor were mixed to fill the annular space. Annular space was pressured to 1100 psi for 15 minutes with no loss of pressure.
1175190456 2007-03-29 Checked injection well. The was no pressure on the annulus side. The gauge on the injection side was broken and could not be removed. Gauge must be replaced. Pressure must be kept on injection or annulus side or else monthly test must be done to a minimum of 200 psi for 15 minutes with less than 5 % loss in pressure. Information must be recorded and submitted to the State. Also there is no ID posted at the well. ID must be posted immediately. Since wells recently changed ownership, new signs are probably needed at production wells and tank battery. Called Tony Kreider and left message to call inspector.

Since this line is on the ground surface, it will not need to be tested since it will be replaced with a permanent buried line when the crops are removed. The permanent line must be pressure tested with the ODNR inspector present before put into operation. Also remind operator that the pit and other disturbed areas around the wellhead need to be reclaimed.
