-1748805332155141 2004-08-05 Reviewed oil well on site for I&O program. The landowner said an application was submitted 10 years ago for I&O when house was built but there is no record in Columbus. Landowner is now considering selling property. Tanks and junk around well were removed at property owners expense. There was no production equipment but the tubing and rods were in place. No sign of contamination around well. Well is within 200 feet of water well and pond.
-847062622 2006-07-13 All equipment removed from site. Well has been cut off below ground and backfilled. Operator said ID plate was attached. It does not appear as though the site has been seeded and cement on ground from plugging needs to be cleaned up.
1152132480 2006-07-05 Witnessed completion of plugging well. The operator attempted to plug the well on 6/30/06 with inspector Sam Faust but in hole obstruction prevented cementing. Obstruction was cleared and well was plugged on 7/3/06 and witnessed by inspector Tom Benko. Operator pumped 125 sacks of cement in well but there was no cement return to the surface on that day. On 7/5/06 the operator determined the cement depth at 476 feet and pumped an additional 75 bags of cement which include 1% CaCl. Cement returned to the surface but then fell to about 10 feet below top of pipe. Additional cement will be mixed and added when pipe is cut off and ID plate attached. Problems occurred in hole when the operator tried to have the 4 inch casing shot to remove the casing. Apparently the 6 inch casing was shot by mistake which may have resulted in an obstruction in the pipe. The 4 inch casing was then shot and the casing removed.
1152543094 2006-07-03 Plug well. Circulate tubing to 952 feet. Six inch casing was shot at 394 feet thinking it was the four inch casing. Shot the four inch casing at 277 feet and pulled casing. Shale / clay fell into well. Tubing was plugged with shale when run in hole, pulled tubing and re ran to 952. Well plugged with cement.
1156968867 2006-08-30 Area around well head is being used for livestock. No cement remaining on ground. Final restoration approved.
