-1345178029 2014-09-03 Upon inspection, it was observed that this well is equipped for production. New identification information has been posted at the well head. I spoke with Mr. Paul Valentine in person, and he stated that he is in the process of acquiring pumping units for all the wells on the property. Mr. Valentine also stated that current production records for all wells on this property will be submitted this month.
1406142843 2014-07-22 Performed an inspection on this well in response to a landowner complaint. Upon inspection , it was observed that this well is equipped for gas production only. The well consists of 6.625" casing with a gas escape packer with 2" production tubing and rods. There is no line attached to the fitting on the 2" tubing. There is a 1" poly line attached to the 1" gas escape tubing feeding into a nearby gathering line. The "scissor jack" pumping unit is still in place and the polish rod is very corroded. The central power unit for the shackle line system is not operational. The 100 bbl production tank for this well is full and has spilled over. The spill appears to have happened some time ago and there is oily sludge around the tank bottom extending approx. 10' away from the tank toward Sheets Run, a nearby creek. There is no dike built around the tank. The tank will have to be pumped down, oily sludge must be removed, and a dike must be built to SPCC standards. Photos were taken of the location to support these findings. There is no identification information posted at the well or production tank. According to RBDMS production records, this well produced 100 mcf of gas in 2012. No production records were submitted for 2013. P&D Valentine Oil Co. will have 14 days upon receipt of this notice to fix these issues and bring the well into compliance. 9-9-05(A)10

1407521290 2014-08-08 Upon inspection, it was observed that no changes have been made to this well location. I spoke with Mr. Paul Valentine in person and he stated that he is in the process of making new ID tags for all the wells on this lease. Mr. Valentine also stated that updated production records will be submitted for all of the wells on this lease. The production tank which had overflowed is still full of fluid and the oily sludge is still present on the ground surrounding the tank. I instructed Mr. Valentine to pump the tank down and clean up the contaminated area as soon as possible. Mr. Valentine agreed to take prompt action on bringing this well into compliance.
