-1144493337 | 2014-03-05 | My inspection found the wells cut off below surface and buried. Annarock has until 8/18/2014 to complete final restorations. I called Rocky, Annarock owner, to remind him to send the original lease road waiver to ODNR.
1739297992 | 2013-03-07 | My inspection found the well not in production. It was equipped with a plunger lift unit and surrounded by small trees and thorns. The tank battery contained one, rusty, 100 barrel production tank. The separator was locked and gate valves closed. There were overgrown weeds, thorns, and small trees surrounding the tank battery and separator. The dike was weathered and not capable of containment requirements. A faded identification sign was posted on the separator. I researched our database and found the last reported production in 2002 followed by zero production from 2003 to 2009. There was no reported production for 2010 and 2011. The lease road was in stable condition and shared by permit number 2-1845. I contacted Rocky at 330-307-7194 and told him I was issuing a compliance notice to plug or produce the well. | NONE
698433922 | 2014-01-30 | I received a call from Rocky of Annarock Petroleum. Rocky stated that Nick's Well Plugging would be moving onto location within 1.5 to 2 weeks to plug the well.
799808633 | 2014-06-04 | I was on location to perform a final restoration. I found during my inspection piping from the production equipment on location. All of the production equipment was removed. There were no pits, Nick's Well Plugging used a half round steel container for all returns. The site was not graded. Vegetation was well established. I called Rocky, owner of Annarock. Rocky stated he was waiting for the area to dry up in order properly reclaim the site.
I instructed Rocky to: Remove all of the flow lines associated with the production equipment. Grade the site. Spread seed. |
