1210i1651954 2001-06-06 06/05/01 received a complaint from the central OEPA. Well site has contributed to oil in creek in past. This date arrived at site with Sam Faust. Company was hot water treating well. Old oil stains around pump jack and old oil at motor end which appears to be motor oil from unit. Evidence that both types of oil have been carried by surface water to the creek. Request operator to clean up this area and to install 3/4 of dike or full dike on lower side for future containment.

1371137063 2013-06-13 Inspected the Cox #1 (#3633) as part of a standard productive well status check. The well is pumping by a gas powered Lufkin pump jack and the well pad is clean and free of excess contamination. The storage is shared with the Cox #2 (#4038) and consists of two tanks (#42616 & #42539) and a series of five brine tanks. A vertical separator sits east of the storage. Location information: The well identification and emergency contact information is marked at the storage. Status: ok.
