1281195595 2014-08-05 Upon inspection, it was observed that this well is equipped for production, and may be producing. The well consists of a pumping unit with a gasoline motor standing over 6.625" surface casing and 2.375" production tubing. All valves are in the open position, and the polish rod is shiny. Gas pressure was detected on the production tubing due to a leak around the threads of a fitting. There is identification information posted at the well, but no API number is listed. RBDMS records show that no production records have ever been submitted for this well. The tank battery consists of a 100 bbl production tank and a small knockout tank with approximately 2' of oily fluid in it. The knockout tank has several lines hooked to it, one of which has rusted and fallen off, allowing the tank to leak fluid. The knockout tank does not appear to be in use, however there is a kill area approximately 6' wide by 40' long running downhill from the tank toward Sycamore Fork, a small tributary to Fifteenmile Creek. Photos were taken of the location to verify these issues. The knockout tank must be removed and the contaminated area must be remediated. Production records for the well must also be submitted. The operator will have 14 days upon receipt of this notice to reach compliance. 9-9-05(A)8

1291111532 2005-09-13 Inspection this day found well with jack producing, tank has good ID's, good dike. This is an older well that they went in and deepened, per the pumper, so am sure I am putting right well with correct number.
