Tom said to monitor the well at this time. There is no evidence of communication out side of the 4 1/2" casing.

1251072283 2011-11-29 Company gauges had seized up and I was unable to remove them for the test. Company's gauges read 385 psi on annulus and 30 psi on injection. The power had just gone out there at the well and the operator was concerned with getting the power back online. I will return within a couple of days to perform the test again and have the operator remove the company gauges.

No dye was observed at the seep location.

The first truck (License number PVT 1149) was not labeled with their UIC number the truck was finished unloading when I arrived. The second truck (License number PVR 3634 ) was labeled with the UIC number UIC #306 (1.75" lettering) and Brine (3" lettering) although the lettering was not 4" in height. I called BD Oil Gathering and told the office staff that their trucks must be labeled correctly for all UIC brine hauling with-in the state. I also informed Dylan Harlow (Green Hunter) that they need to make sure that the trucks that are hauling to their facility are registered and labeled for brine hauling.

There was no brine label on this truck. I gave the drive a copy of the brine hauler requirements. I collected a brine sample from the first truck. The brine sample was taken to the Jackson office for shipment to the ODNR Lab.

156246569 2012-08-02 The gauge was too tight to remove, even with a back-up. When I attempted to remove the gauge from the 1/4" valve to check the annular space between the 2" tubing and the injection tubing I noticed it too was especially tight. In an attempt to apply enough pressure to remove the gauge I broke the 1/4" nipple at the point where it threads into the bull plug reducer. A 2" ball valve was closed behind the broken nipple so no pressurized fluids escaped the well. I called the company and they will be replacing the equipment tomorrow.
