-1120332088 2010-09-14 This status check was requested by Jay Cheslock. I performed a field inspection and walked around both the wellhead and the tank battery. This inspection satisfies 1501:9-9-05. There were no violations at this time.
-1627923124 2012-04-24 On 4/24/2012, I completed a follow-up inspection for DNR Inspector Chris Grimm. My inspection found that the well tender for Eric Petroleum had removed the damaged brine tank and installed a bull plug into the brine flow line tee. I called the field supervisor, Rick Huffman, at 330-904-2576, to inquire if a new tank was going to be installed. Rick said there were no plans for installing a new brine tank, instead the one 100 barrel oil storage tank would be used for both the oil and brine. There are no further violation, inspection passed.

1890149193 2012-04-20 FOLLOW UP TO NOV # 854407023 - On 4/20/2012, I completed a production well inspection status check for DNR Inspector Chris Grimm. This well shares a lease road with permit numbers 2-2831 and 2-3045. My inspection found a 12" hole in the side of the 30 barrel brine tank. I called Rick Huffman, field supervisor for Eric Petroleum (330-904-2576), to inform him of the violation. Rick said he would send out the well tender immediately to replace the brine tank . I will perform a follow-up inspection by 4/27/2012.
