1352931588 | 2012-11-14 | I received a call from Steve Pflugh from Stark Credit Union regarding the condition of the Hawkins 1 well (34151247080000) located at 1235 West Beech Street, Alliance, Ohio. I met Curt Lanzer from Quality Oil and Gas at the Hawkins 1 well site on November 14, 2012. My inspection showed the lease road was in good condition, graveled to the tank battery, and does not show signs of erosion. The tank battery consists of one 100 bbl stock tank, one 50 bbl brine tank, and one mechanical separator. There is a chart at the site with an installation date of November 13, 2012 showing 90 psi on the flow line. The 100 bbl stock tank has the proper identification sign attached. The drain line and brine discharge line on the 100 bbl stock tank includes bull plugs but does not have locks. The brine discharge line bull plug has a brass valve attached with no lock attached. The 50 bbl brine tank has a bull plug but no lock. The 100 bbl tank does not have a spark arrestor installed. The tank battery is secured by a chain link fence and the access gate is locked. The dike has good walls and containment capacity with minor vegetation growing inside. All valves on the flow line from the wellhead to the tank battery are open, indicating the well is not idle. There are no unburied flow lines between the wellhead and tank battery. The wellhead does not show signs of oil, gas, or saltwater leaks at this time or any indication of past leakage. At the time of inspection, no odors were detected using a gas meter. The fencing surrounding the wellhead is in good condition but the gate is not locked and there is no identification sign attached. There is no pumping unit needed at this well. The pressure gauge on the tubing string had 260 psi and the pressure gauge on the 4.5 inch casing string had 450 psi. The tank battery is greater than 100 feet from the nearest inhabited dwelling, greater than 50 feet from the nearest road and greater than 50 feet from the well head. The 100 bbl stock tank is greater than 10 feet from the separator and greater than 3 feet from the brine tank. The mechanical separator is greater than 100 feet from the nearest inhabited dwelling, greater than 10 feet from any tank, and greater than 50 feet from the well head. I discussed the inspection with Mr. Lanzer and requested that he install locks on all valves associated with the 100 bbl stock tank and 50 bbl brine tank. Plug or lock the brass valve installed on the brine discharge bull plug for the 100 bbl stock tank. Install a spark arrestor on the vent of the 100 bbl stock tank. Attach an identification sign at the wellhead and lock the access gate for the wellhead. Mr. Lanzer will be given 60 days (January 14, 2013) to remedy the findings from this inspection, no violations were issued. | ||||
