-281231193 2011-05-09 Checked on status of plug job. The well location was not accurate therefore, a GPS reading was obtained from this inspection.
-286096618 2012-08-23 I inspected this location today with Brent Bear. The inspection revealed the well location is in non-compliance.
-578088307 2012-08-23 A follow-up inspection conducted with Mr. Brandon Mitchell revealed non-compliance however, the company has performed work since the initial inspection. The steel tanks have now been removed, earthen dike graded, and vegetative cover established. The lease road did not contain vegetative growth. Additionally, there was no application of material such as, seed and straw/hay to establish vegetative growth along this road. A section of 2" steel line was also located along the lease road. Mr. Mitchell will consult with well owner regarding the results of this inspection.
1340823711 2012-06-27 The Division received a complaint alleging delinquent final restoration on property. An inspection revealed that the final restoration does not meet ORC 1509.072. During the inspection, it was noted that oilfield such as, (2) 100 bbl tanks, separator, pumping unit, electric line and junction box, and production line(s) were still exist on property. The lease road, earthen dike and well location will also need to be restored and vegetation established. At 5:00 PM, on 6/26/2012, and prior to this inspection, the well owner was verbally instructed to complete final restoration within 30 days. If the well owner fails to comply with verbal notification a written notification will be issued. NONE3
