1057020855 2003-06-30 Inspect well. Well is capable of production and product lines are under pressure. Site "Cleanup" has occurred in some capacity. A front end loader bucket may have been scooped from well head area and replaced or covered with sand fill in the immediate vicinity of the well head. A bucket of soil/ material was dumped near the top of the downward slope covering part of a heavily oil stained area, then the bucket was back dragged to continue covering the oil stained area. The oil run off area has not been remediated or changed from when oil originally flowed over the slope. Some oil residual oil is still present in the run-off channel. The area around the well has been worked with a front end loader when the ground was wet, and subsequently has dried. The presence of oil has been covered and is not visible.

Inspected tank battery for wells 693, 694, and 689. An uncontrolled release of brine and or petroleum has occurred from these tanks or the associated piping. The release flowed around each of the tanks staining the gravel base and flowed along the access road, then crossed the access road and pooled at the base of 6 to 8 40 foot pine trees that are currently dead. Attempts have been made to cover the stained ground with pea gravel and other debris.
