-1010140923 2012-08-14 On 8/14/2012, I conducted a follow-up inspection and found chain link fence poles were in place and the proper identification posted. I called Bill Templeton (330-240-5573) and was informed he was still working on finishing the chain link fence and getting the proper identification posted. I will perform a follow-up inspection by September.
-1342786349 2012-08-27 I conducted a follow-up inspection. My inspection found the tank battery surrounded by a chain link fence and the area around the well head has been grated. I spoke with Dustin Energy and the chain link fence will be installed around the well within two weeks.

-398353096 2012-06-05 On 6/5/2012, I investigated a complaint with DNR Steve Ochs. According to Horvath, Dustin Energy, Inc never reclaimed the well site by not spreading a pile of topsoil located behind the landowner's house. My investigation found there was a pile of topsoil not reclaimed, the area around the well was overgrown with tall weeds, and tank battery along with the well head had no fencing or proper identification. I called Bill Templeton of Dustin Energy, Inc at 330-240-5573 to discuss the complaint and urban requirement violations. I instructed Dustin Energy to install the proper identification and we agreed that all fencing needed to be in place by the end of August. Bill said he would work with Mr. Norvath to find a day to spread and grade the top soil.
1342188005 2012-07-12 On 7/11/2012, I received a call from Mr. Horvath concerning the reclaimation of the well on his property by Dustin Energy, Inc. My inspection found the dirt pile behind the landowner's house was still there. I called Dustin Energy to ask about getting the dirt pile spread, the area around the well graded, and seeded, also I gave Bill Templeton of Dustin Energy

506557190 2012-07-11 I received a call from Mr. Horvath concerning the reclaimation of a well on his property owned by Dustin Energy, Inc. My inspection found a pile of topsoil behind the landowner's house that had nevery been reclaimed after the completion of the well. I called Dustin Energy and told them to finish the reclaimation by spreading the topsoil and reseed. I reminded Dustin Energy that I told them on 06/05/2012 that a fence needed to be installed at the tank battery and well head by the end of August.
