-1127959277 2005-10-12 The well was not equipped for annular disposal. The well pumped to a common tank battery.
-1418470761 2012-09-28 I spoke with the field supervisor of American Natural Gas and he explained the issues with the lines at the well. The rig is still on site at the well. Once the issues are corrected the issue with the identification will be addressed.
-480953407 2012-08-22 The contaminated soils have been removed from the site and a service rig has been moved on the site to perform work on the well. The identification tag is still not complete.
-58479191 2007-08-30 The well was not equipped for annular disposal.
-923675336 2013-02-27 Follow up to 1420244921. The contamination has been removed, and the well head is visible, but there is no identification at the wellhead. There is an ID at the tank battery but it is has multiple wells plumbed into it. Each well head is to be identified when multiple wells are being plumbed into a common tank battery.
-969010333 2013-06-13 Follow up to 1420244921. No contamination, ID on well, surface casing visible.
1340721927 2012-06-26 Surface casing visible and well is not set up for annular disposal. None of the other violations have been fully addressed.
1420244921 2012-05-30 Well equipped with a pumping unit and electric motor. The identification tag at the well listed the incorrect owner. The valves on the tubing and the production were open. The stuffing box was leaking and had contaminated an area 5x5 feet around the wellhead with oil. The area 15x10 feet around the wellhead was void of vegetation. The paraffin from the leak, the contaminated soils, as well as peat moss piled up around the wellhead made it impossible to view the surface casing head. Oil and contaminated soils around the wellhead shall be removed and disposed of properly. Any affected area shall be seeded and mulched and steps shall be taken to establish vegetative cover. All producing leases shall be legibly identified in a conspicuous place on or near the well-head or the storage tank's) as to owner, lease name, well number, permit number where available, county, and an emergency telephone number. If multiple wells are being produced into the common tank's) each well-head shall be identified as to owner, permit number where available, well number and lease name. Any change of ownership shall be shown at the well-head or storage tank's) not later than sixty days after the date of the assignment or transfer. 9-1-07
