I talked with Jimmy Ray (Richfield) He gave me the information on the plug back depths and will fax me a copy of the cement ticket for the plug back.

The gravel access road has been left in place. Mr. Martin has signed a waiver form for the road to be left in place, and for the stock piling of gravel of farm use.

-492016748 2008-02-27 Drilling horizontally today. Pit looks good. Pit has a orange safety fence around it. Access road is well built and has stone on it. Due to the heavy rain the well site is muddy. The flow line is only 30' to the pit. I informed Merl Richards of the requirement of 60' of flow line while drilling on air. I gave Merl a copy of Ohio Oil & Gas Rules & Laws. Also gave him a copy of the BMP for well site construction.

They want to use the pit for another well and for frac job. Pit is constructed of red clay under the liner. Pit water level does not seem to be lowering due to the liner failing.
-980194491 2008-02-28 I called Merl Richards and told him that Joe Hoerst said the pit water is produced water and it must go to an injection well. Merl informed me that they are preparing to plug back for the kick off today.

I gave Merl Richards a copy of The Oil and Gas Laws and Rules, and a copy of the BMP for well site construction. I also pointed out that their flow line is short and I explained about the 60' requirement for flow lines when drilling on air. Their flow line was approximatly 30'. Merl asked me if they could have the pit water tested and if it was good could they spread it on the ground. I told him that I would ask my supervisor and get back to him. They are to store the water in tanks for now.

1262826134 2009-07-23 Ran tubing to 2675'. Circulate water and oil back to surface. Pump 1st plug from 2675' to 2130' in three stages two 200' plugs and one 100' plug. Cement pump truck broke hydraulic line.
1262831607 2009-07-27 TD TOC plug at 1824'. Pump 3rd plug 1824' to 1728'. Pit liner failed. Had to close pit and construct new pit. New pit has felt liner and plastic liner over the felt. New pit sides are tapered.

A single mechanical separator and gas meter is on location. Pit has been closed. No seed or mulch has been applied to location.
