-1373082375 2010-10-04 A follow-up inspection conducted with Mr. Tommy Thompson, Wayne National Forest, revealed non-compliance. Two electric poles with attached line remain at the location. Per Mr. Thompson, the electric poles can be cut down and left however, the line must be removed from location. Section of plastic line approx 1.25" observed at creek crossing and 2" steel line found near the p/a well location will need to be removed. Material required to be removed have been marked for company. Notified Mr. Doug Smith of inspection results and requirements.
-153641448 2010-12-23 A follow-up inspection revealed non-compliance. The steel lines located at front entrance appear to be Columbia C106 lines. Located to the left of the entrance (west), two separate sections of 1.25" orange plastic line remain in the woods beginning near the creek bank. To the right of the entrance (east), 1.25-1.5" steel line remain on the property. The line is routed across the creek and continues towards the east. The line routed in a easterly direction cross creek away from the location. New photographs were obtained from the inspection. Note: snow cover at time of inspection. Mr. Robert Stonerock, Oil & Gas South Region Supervisor will be notified of final restoration status. Mr. Tommy Thompson with the Wayne National Forest will also be notified of the inspection results.
-1732362308 2009-07-15 An inspection revealed that the Final Restoration does not meet. Two electric poles with line remain on the location and will need to be removed. Called Mr. Chip Smith at 1:49 pm today and advised of inspection results. NONEA

A follow-up inspection to verbal notifications revealed that the Final Restoration does not meet ORC 1509.072. A voicemail message was left for Mr. Doug Smith today at 11:57 a.m. following the inspection. All production equipment will need to be removed from the location. Please address the following: Remove all electric line. Cut the (2) electric poles and leave for wildlife habit, or remove per Wayne National Forest representatives.
