-1201380406 2010-06-14 An inspection was requested by Central Office to verify annular disposal connection. The current coordinates for this well revealed no well location. The closest well contained an identification tag stating Core #2. No annular disposal connection was observed at this well. A GPS reading was obtained for this well. +/-14 39.10868 -82.06596. The obtained coordinates measure approx 300' from both plotted locations for Core #1 and #2 wells. At time of inspection, RBDMS showed a total of three (3) Core wells listed as PR. Contacted Mr. Spence Carpenter, Field Supervisor, with JD Drilling. Mr. Carpenter stated only two (2) wells existed on this lease. Further stating that well in question is referred to as the #2. Notified Mr. Mike Williams and Steve Opritza at Central Office via email.
